US VISA PHYSICAL EXAMINATIONS

Please note that validity of US VISA physical exam report is 6 months from the date of exam.
If any abnormality is found, its validity is 3 months from the date of exam.

Appointments are available from Monday to Friday from 9:30 am to 11:00 am and from 1:30 to 3:00 pm, except National Holidays. Applicants who are pregnant or possibly pregnant are requested to inform the receptionist in advance. The average time needed is about two hours. Please be dressed in a manner that allows you to dress and undress easily.

Vaccinations:

The vaccinations required will depend on the age of the applicant. See immunization charges below

It is possible to take an antibody test prior to deciding which vaccinations are necessary. However, it takes about 3 working days to get the results of this test( the fee of this test is JPY4,000 each) and this is therefore NOT an option for those wishing to have the 'Urgent Service. If the results show that you are not immune to certain diseases, you will need to be vaccinated.

Please bring the following items on the day of examination:

1) Case Number Letter (Letter from US Embassy)

2) Valid passport.

3) One photograph (passport size) with your signature on the reverse side.

4) Vaccination record, if any.

Note. Refugees and adopted children aged 10 and under DO NOT need vaccinations.

5) Glasses or contact lenses (if worn).

6) If you are provided with "Alien Case Number", please bring the document with you.

7) If you are taking any medication prescribed by a doctor, please bring a list of the medication.

8) If the examinee is less than 6 years old, please bring "Mother & Child Notebook" if any.

Payment can be made by Cash, or VISA/Master, JCB, Amex, J-Debit cards.


For Female Applicants:

If you are pregnant or possibly pregnant, please inform the receptionist when you book your appointment. Pregnant applicants need to bring a pregnancy certificate issued by your gynecologist. They are then exempt from certain vaccinations. However, you must undergo Chest X-ray. Please consult your gynecologist before considering taking this exam.

Results :

Your report will be ready according to the schedule below. The report will be ready if your vaccination requirement is met and your laboratory and Chest X-ray results are normal. Please note that if you choose to take antibody tests or to come back for the vaccinations at a later date, the report cannot be ready according to the schedule below.

US Visa (2-14 years old) 17,850yen  (In the case of Tuberculin Test result is positive, additional fee for chest X-ray will be charged.)
