Private Annual Physical Exam.

The set plan is made according to your gender and age. For more details and appointment, call 03-3432-6114. You must fast for 12 hours prior to the examination, and you will receive the completed report in 10 days to 3 weeks' time, depending your attending doctor.

If there is a form provided by your insurance company, please fax it to 03-3432-5980 because we need to know the required test items in advance. If you want us to send a bill to your insurance company, we need a payment authorization letter from the insurance company in advance.

School Physical Exam.
(Mon to Friday: 9:30 a.m.- 3:00 p.m. / Sat. 9:30 - 11:20 a.m.)
This can be done on Monday to Saturday. If the examinee is less than 17 years old and
immunization is required, we ask a parent to accompany the examinee,
as we do not immunize patients less than 17 years of age without
the parent's authorization.
