NEW ZEALAND VISA PHYSICAL EXAMINATIONS


Appointments are available from Monday to Friday from 10:00 am to 11:00 am and from 1:30 pm to 3:00 pm. The average time needed is about two hours. Please be dressed in a manner that allows you to dress and undress with ease.

Applicants who are obese or have high blood pressure may need to undertake additional tests.

12hrs fasting prior to the appointment time is preferable. (You may only drink water during the fasting time.)


  Results:
The reports will be completed within 1 week (if there are no abnormal findings). If only Chest X-ray was taken, the report will be ready on the next business day around 5pm. The reports can be mailed to your home address ( mailing fee: \530(expack) or \1,050(Express mail) ) or you may come and pick up the reports.


  For Female Applicants :
If you are menstruating on the day of the physical examination, you must come back to give a urine sample 2-3 days after your period has finished. Therefore, if possible, please make your appointment outside of these times.
This is not applicable to applicants who only need to take Chest X-ray.


  For Pregnant Applicants:
If you are pregnant or possibly pregnant, please inform the receptionist when you book your appointment. Pregnant applicants need to bring a certificate stating that they are pregnant. They are then exempt from Chest X-ray. A pregnancy test can be performed at this Clinic for examinees who do not have a sufficient proof of pregnancy. The cost of this test is 5,780 yen.


  Please bring the following items on the day of examination:
1) Valid passport

2) 3 photographs (passport size) with your passport signature the reverse side.
Only 1 photograph is sufficient for applicants who undertake only Chest X-ray.

3) Previous Medical Reports from a doctor for those who have a history of TB or other major diseases. (The report should include diagnosis, date and period of treatment, details of medication)

4) If you are taking any medication prescribed by a doctor, please bring a list of the medication.

5) Glasses or contact lenses (if worn). Both corrected and uncorrected vision need to be tested, therefore contact lenses users are requested to bring a lens case and lens solution.

6) If the examinee is less than 6 years old, please bring "Mother & Child Notebook" if available.

7) Physical Examination Forms. Form 1007 for full medical or Form 1096 for chest xray only.

Please note that 4) and 5) are not applicable for examinees who undertake only Chest X-ray.

Payment can be made by Cash, or VISA/Master, JCB, Amex, Diners, Discover,J-Debit cards.


  Fees:
Examinees who have Form NZIS 1007 (Full medical exam including laboratory tests)
Age 0-10  :  10,500 yen
Age 11-14  : 16,800 yen
Age 15 and over :  39,900 yen
Pregnant(Age15<) : 33,600 yen

Examinees who have Form NZIS 1096
(Chest X-ray Only): 11,550 yen




 Test Requirement:
Age 5 −10: Urine test
Age 11−14: Urine test + Chest X-ray
Age 15 and over : Urine test + Chest X-ray + HIV blood test + Syphilis blood test +
            Complete blood count test + Hepatitis B surface antigen blood test +
            Liver function blood test +Serum Creatinine blood test
