IN TOKYO'S 23 WARDS】

Vaccinations subsidized by the Japanese Government are available at our clinic.
If you have a coupon from your ward office, these are FREE of charge.
Certain other criteria need to be fulfilled (see below).

・Application requirements (The recipients must)
①Be resident one of Tokyo's 23 wards（registered as a local citizen）
②Be within the correct age range for the relevant vaccination

・Inoculations subsided by the government (free)
For residents of Tokyo's 23 wards
(All Japanese routine vaccinations except BCG)

（Measles and Rubella） ②5yr-up to 7yr, before entering

Japanese Encephalitis ①6mo-up to 90mo （3 times）

　 (1-3 times=incompleted number of dose）
　　　 ②9yr-up to 13yr （once）　

For residents of Minato ward only

You must bring your Vaccination coupons(yoshinhyou) and Mother and childbook(bosihtechou)
If you do not bring coupon and book, you will have to pay for the shots.
With regard to the vaccination coupons, if you do not have them for some reason,
please make an appointment after receiving the coupons.
They can be re-issured if you contact to the Health Center at your ward office directly.
Please fill out the coupon before having the shots.
