Appointments are available from Monday to Friday from 9:30 am to 11:00 am and from 1:30 pm to 3:00 pm. The average time needed is about two hours. Please be dressed in a manner that allows you to dress and undress with ease.
Applicants who have high blood pressure will need further blood tests and a cardiologist report.
Applicants who have diabetes, kidney dysfunction, etc. will also need additional blood tests.
Applicants who have had hepatitis or tatoo may need Hepatitis-B and Hepatitis-C
Antibody tests.
Results:
The reports will be completed in 2 to 3 working days (if there are no abnormal findings) and immediately dispatched to the Immigration Office in Manila,Philippines.

Mailing fees:
EMS (Post Offices Express Business Mail): JPY 2,000 per family.
FEDEX (International courier): JPY 10,000 per family.
For Female Applicants:
If you are menstruating on the day of the physical examination, you must come back to give a urine sample 2-3 days after your period has finished. Therefore, if possible, please make your appointment outside of these times.
If you are pregnant or possibly pregnant, please inform the receptionist
when you book your apointment. Pregnant women are requested to bring a
certificate which proves that they are pregnent.
Please be sure to bring the following with you:
1) A valid passport.
2) A letter from the Canadian Government, which shows your File No.
OR
If you are in the category of EFC (Exempt Family Class), please be sure
to bring the Appendix-C form. Please specify the "visa office"
where your sponsor is to submit the visa application documents.
Please also make sure to clarify the destination of your medical report.
Canadian Embassy in Tokyo does not deal with medical reports. The destination must be outside Japan. We also
need the address and telephone number of the destination if the destination
is not Medical Office in Manila, Philippines. Please note that we cannot
send the report to patients, we can send it to Canadian Medical Office
outside Japan only.
3) 4 photographs (passport size) with your signatures on the reverse side.
EFC applicants will need to bring 5 photographs.
4) Glasses or contact lenses (if worn). A corrected vision should be better
than 0.5 or 20/40 or 6/12.
5) Examination fee. Payment is in cash,
we also accept VISA/ MASTER/ JCB/ AMEX/ J-Debit Cards.
Fees:
Age 15 and older 30,000 JPY
Age 11-14 20,000 JPY
Age 5-10 17,000 JPY
Age 4 and younger 15,000 JPY
5% tax will be added to the above.
