Tokyo Medical & Surgical Clinic is now authorized to perform E-Health medical and x-ray examinations.

Appointments are available from Monday to Friday from 9:30 am to 11:00 am and from 1:30 to 3:00 pm, except National Holidays.
The average time needed is about two hours.

Health Sector employees and Medical Practitioners need to undergo additional blood tests.

Applicants who have had blood transfusion or tatto will also need additional blood tests.

Please advise the receptionists accordingly when booking your appointment.

Applicants aged over 70 applying for sightseeing Visas are also asked to make this clear to the receptionist at booking.

If you are applying through E-Health, please bring the "Referral Letter": You do not need to bring your photo, because we will take your photo at this site. Please advise the receptionists accordingly when booking your appointment.

Results:

l The reports will be completed within 2 working days and delivered to the Embassy by hand on Monday, Wednesday and Friday (excluding public holidays).

l Those reports which require additional tests will be delivered within 5 to 6 working days.

l For those who need the report urgently, we have a Same-Day Service. Applicants must attend before noon, and the report can then be delivered to the Embassy on the same day by Bike courier. The additional fee for this is JPY 8,000.

For Female Applicants:
If you are menstruating on the day of the physical examination, you must come back to give a urine sample 2-3 days after your period has finished. If possible, please make your appointment outside of these times.

If you are pregnant or possibly pregnant, please inform the receptionist accordingly　when booking your appointment. Pregnant women are requested to bring a certificate which proves that they are pregnant.

Please be sure to bring the following with you:

1) A Valid passport.(A Copy is not acceptable).

2) Physical Examination Forms: Either from the Embassy or a print-out if you are applying on-line.

3) Two photos (passport size) with your name on the reverse side.

4) Glasses or contact lenses (if worn).A corrected vision must be better than 0.5 or 20/40 or 6/12.

5) Previous Medical Report from a doctor for those who have a history of TB or other major diseases.
(The report should include diagnosis, date and period of treatment, details of medication)

6) If the examinee is less than 6yrs old, please bring a Mother & Child Notebook (Boshi-Techo).

7) Examination fee. Payment is in cash, we also accept VISA/ MASTER/ JCB/ AMEX/ J-Debit Cards.

Fees:

For Temporary/Student visa

Age 11 and over: JPY 20,000

Age 10 and under: JPY 10,000

For Permanent visa

Age 15 and over: JPY 25,000

Age 11 - 14: JPY 20,000

Age 10 and under: JPY 10,000

For X-ray Only: JPY 11,000

Pregnant applicants require additional tests: from JPY 5,100

Medical practitioners require additional tests: from JPY 12,500

l 5% tax will be added to the above.
