This clinic is designated by the Japanese Government to administer H1N1 vaccines.
Therefore, we administer this vaccines according to the direction of the Japanse
and Tokyo Metropolitan Goverment.
Please check our website "H1N1 SHOT information" occasionally,
we will update any changes to the Government's policy or rules.

【HOW TO MAKE AN APPOINTMENT】
APPOINTMENTS CAN BE MADE VIA THE DIRECT LINE OR THE REGULAR CLINIC NUMBER !!!

Please call the direct line 03-3436-6025 (available 10:00-13:00 weekdays only)
                                 or 03-3436-3028 (available 8:30 - 17:00 weekdays only)

Please make sure to make an appointment in advance.

We will administer H1N1 vaccines WEEKDAYS ONLY (10:00-12:00 and 14:00-16:00)

【NOTICE】
・Children under 16 years old need to be accompanied by a guardian.
・Under the age of 13 are eligible and recommended to have the 2nd vaccination the interval
  should be 1-4 weeks.   (Having 4 weeks interval is thought to achieve a better immune effect)

【WHAT IS THE COST】
The fee for the vaccine and the doctor's consultation for the purpose of taking the vaccine is
\3,600 including tax.
When you have the 1st and also 2nd shot in TMSC, the fee of the 2nd shot will be \2,550.
If the vaccine is not given, no fee will be charged.


【MINATO-KU RESIDENTS】
・The Minato-ku used to offer subsidy for the vaccination to persons who fall into the category of
  the given priority of the vaccine, see above (A)-(I).  But no longer available offen. (Aprill)


【CAN I HAVE OTHER VACCINES AT THE SAME TIME】
・Following Japanese Government directions:
Seasonal influenza shot may be given at the same time or must be saparated by 1 week.
Measles, mumps, rubella, varicella should be separated by 4 weeks.
all other vaccines should be separated by 1 week.


【REQUIRED FORM FOR APPOINTMENT】
・There is a different form among age groups you belong to that
　you need to download and bring on the day.
Please make sure to print out and fill it out before coming to the clinic.

*Born before April 1, 1994. 　　　==> Sheet 2
(age of Japanese high-school students and older)


*Born between April 2, 1994 and April 1, 1997.　==> Sheet 4
(age of Japanese Junior high-school students)
　

*Born after April 2, 1997.   ==> Sheet 3
(age of Japanese Elementary school students and younger)
